Description
A patient-reported outcome measure used to assess health-related quality of life in patients undergoing cancer therapy. The FACT-G constitutes the core of all subscales of the FACT and can be used with patients of any tumor type. It is comprised of four subscales: physical well-being (PWB; 7-items, score range 0-28), social/family well-being (SWB; 7-items, score range 0-28), emotional well-being (EWB; 6-items, score range 0-24), and functional well-being (FWB; 7-items, score range 0-28). Users of the FACT-G are able to generate an overall score and four subscale scores with ranges and distributions that are sample-specific. .

Description
Functional Assessment of Chronic Illness Therapy-Tuberculosis (FACIT-TB) adds a set of items assessing QoL issues not sufficiently covered by the core FACT-G items, a set of 20 items referring to disease symptoms related to the site of infection (Pulmonary TB); side effects and other issues related to treatment; and additional QoL dimensions such as fatigue, fear of disease transmission, and economic burden of the illness were included in the additional subscale of FACIT-TB.

Description
The JAMAR (developed by Filocamo et al 2011) is a parent/patient-reported outcome measure that enables a thorough assessment of the disease status in children with juvenile idiopathic arthritis (JIA). It includes 15 items that assess well-being, pain, functional status, health-related quality of life, morning stiffness, disease activity, disease status and course, joint disease, extraarticular symptoms, side effects of medications, therapeutic compliance, and satisfaction with illness outcome. The JAMAR is proposed for use as both a proxy-report and a patient self-report, with the suggested age range of 7-18 years for use as a self-report.

MSQOL-54

Description
The MSQOL-54 is a multidimensional health-related quality of life measure that combines both generic and MS-specific items into a single instrument. (Vickrey et al, 1995) (Vickrey et al, 1997) The developers utilized the SF-36 as the generic component to which 18 items were added to tap MS-specific issues such as fatigue, cognitive function, etc. This 54-item instrument generates 12 subscales along with two summary scores, and two additional single-item measures. The subscales are: physical function, role limitations-physical, role limitations-emotional, pain, emotional well-being, energy, health perceptions, social function, cognitive function, health distress, overall quality of life, and sexual function. The summary scores are the physical health composite summary and the mental health composite summary. The single item measures are satisfaction with sexual function and change in health. PROMs for spondyloarthritis

Description
Patient reported outcome measures (PROMs) are questionnaires patients complete on their health and quality of life. The information collected from PROMs can help to monitor patient progress, help professionals and patients discuss health issues with each other and/or help to improve the quality of health services. This PROMs was developed for use with patients suffering from spondyloarthritis diseases. One side includes 10 activities of daily living (translated CASQ-FI), 10 items to assess quality of life (translated CASQ-QoL), VAS for spinal pain, joint pain, global status, fatigue, duration of morning stiffness, review of the systems, falls and cardiovascular risks, self-helplessness as well as self-reported joint and soft tissue pain.

Description
The Functional Assessment of Chronic Illness Therapy - Spiritual Well-being (FACIT-Sp) was developed in the United States to assess assesses cancer patients’ HrQoL and spiritual well-being.
