It is a questionnaire for parents composed of 15 items related to a child’s performance in various everyday activities, with an administration time of 10−15 min. The questionnaire is designed to assess three factors: control during movement (CDM), fine motor /written motor skills (FM), and general coordination (GC). Parents are asked to compare their child’s coordination to that of other children of the same age and rate it on a 5-point Likert scale ranging from Not at all or A little to Moderately or Strongly - like my child.
