Description
A comprehensive needs assessment tool for use by professionals working with the elderly. It was developed from the Camberwell Assessment of Need (CAN) to incorporate the special needs posed by the elderly. Twenty-four areas of individual need are assessed by the CANE, as well as two questions to assess the needs of the person’s carer.A summary of met and unmet needs is then produced from the information gathered, which can lead directly to possible interventions and care plans.

Description
A self-administered questionnaire developed by the Public Health Agency of Canada (PHAC) that identifies people at high risk for developing type 2 diabetes.
