Description
A 25-item measure using four scales containing information about the disease (4 items), information about medical tests (3 items), information about treatment (6 items) and information about other services (4 items). In addition to eight single items relating to self help, different places of care, the provision of written and/or digital information, satisfaction with the information received, desire to be provided with more or less information and usefulness of the information received.

Description
A liver disease-specific questionnaire that contains 24 items, which investigate the severity of symptoms among patients with liver disease. Nine items assess the severity of symptoms: itch, joint pain, right upper abdominal pain, sleepiness during the day, worry about family situation, decreased appetite, depression, fear of liver disease complications and jaundice. The other nine items assess how the symptoms influence patients’ daily activities. The last six items measure the severity of memory problems, change in personality, hindrance in financial affairs, daily time management, decreased sexual interest and decreased sexual activity.

The REALM-R is a brief screening instrument used to assess an adult patient’s ability to read common medical words. It is designed to assist medical professionals in identifying patients at risk for poor literacy skills. The REALM-R is a word recognition test – not a reading comprehension instrument.
