Description
A measure of five dimensions of quality of life in bone marrow transplant patients. FACT-BMT is a combination of two tools; functional assessment of cancer therapy-general (FACT-G) which assesses the effects of cancer therapy in the four major areas of physical, social/family, emotional and functional well-being and the BMTS assesses specific BMT related concerns .

IPQ

Description
The IPQ was developed to measure illness perceptions in a variety of illnesses. This questionnaire assesses perceptions on each of the five dimensions by asking patients for their own beliefs about their condition; The major cognitive components identified from research are: Identity - which is comprised of the label of the illness and the symptoms the patient views as being part of the disease; Cause - personal ideas about aetiology which may include simple single causes or more complex multiple causal models; Time-line - how long the patient believes the illness will last. These can be categorised into acute, chronic or episodic; Consequences - expected effects and outcome of the illness; and Cure/control - how one recovers from, or controls, the illness.

Description
A self-administered, disease-specific measure developed with reference to the Japanese editions of SF-36, and the Roland-Morris Disability Questionnaire (RDQ) to assess health-related quality of life. Description
The KDQOL-SFTM version 1.3 combines the generic SF36 instrument with the kidney disease specific instrument. The questionnaire consists of 80 items divided into 19 dimensions. The disease specific component of KDQOL-SFTM 1.3 includes 43 kidney disease targeted items. They comprise 11 domains, including: symptom/ problem list (12 items), effects of kidney disease (8 items), burden of kidney disease (4 items), cognitive function (3 items), quality of social interaction (3 items), sexual function (2 items), sleep (4 items), social support (2 items), work status (2 items), patient satisfaction (1 item), and dialysis staff encouragement (2 items). Instrument assesses one of the psychosocial stressor of social isolation; marginality, the distance from the centre of decision making, . The concept of marginality describes the social margins that bring about social isolation as well as the psychological isolation of individuals. The NEI-VFQ is a broad, vision-targeted questionnaire used in the assessment of visual impairment.
