COPCORD

Description
The World Health Organization (WHO) and the International League Against Rheumatism (ILAR) initiated the Community Oriented Program for the Control of Rheumatic Diseases (COPCORD) project in 19814 . The aim of the program was to collect data on the burden of illness brought about by rheumatic disease and to institute preventive measures such as educational interventions. A number of countries have participated in this program and have used the WHO-ILAR COPCORD Core Questionnaire (CCQ) to determine the prevalence rates of rheumatic diseases in their countries. The English version of the CCQ includes questions on 7 domains: pain, symptoms (tenderness, swelling, and stiffness), trauma, functional ability, coping, health-seeking behavior, and treatments received. DCS

Description
The Depressive Cognition Scale (DCS) is an eight-item instrument that measures hopelessness, helplessness, powerlessness, purposelessness, worthlessness, loneliness, emptiness, and meaninglessness (Zauszniewski, 1995). Based on Beck’s cognitive theory of depression and Erikson’s theory of psychosocial development (Zauszniewski, 1995), the DCS assesses depressive cognitions that may stem from the unsuccessful resolution of Erikson’s eight developmental stages (Zauszniewski, 1995). The instrument is a selfadministered eight-item scale scored on a 6-point Likert-type scale ranging from 0 = strongly disagree to 5 = strongly agree. ( Zauszniewski and Bekhet, 2012).

Description
Bond and Tichy (2007) originally developed the Disaster Preparedness Evaluation Tool© to assess the preparedness of Nurse Practitioners for disasters. The instrument was designed to measure Nurse Practitioner’s knowledge and skills regarding disaster/post disaster response and management.The DPET consists of 68 items; 25 items relate to predisaster preparedness, 16 items relate to mitigation, six items relate to the debriefing stage of a disaster response and 21 are openended questions regarding demographic data.

Description
The Duke University Religion Index (DUREL) is a five-item measure of religious involvement, and was developed for use in large cross-sectional and longitudinal observational studies. The instrument assesses the three major dimensions of religiosity that were identified during a consensus meeting sponsored by the National Institute on Aging. Those three dimensions are organizational religious activity, non-organizational religious activity, and intrinsic religiosity (or subjective religiosity).

Description
A patient-reported outcome measure used to assess health-related quality of life in patients undergoing cancer therapy. The FACT-G constitutes the core of all subscales of the FACT and can be used with patients of any tumor type. It is comprised of four subscales: physical well-being (PWB; 7-items, score range 0-28), social/family well-being (SWB; 7-items, score range 0-28), emotional well-being (EWB; 6-items, score range 0-24), and functional well-being (FWB; 7-items, score range 0-28). Users of the FACT-G are able to generate an overall score and four subscale scores with ranges and distributions that are sample-specific. .
