Description
The Drug Abuse Screen Test (DAST-10) was designed to provide a brief, self-report instrument for population screening, clinical case finding and treatment evaluation research. It can be used with adults and older youth. The DAST-10 yields a quantitative index of the degree of consequences related to drug abuse. The instrument takes approximately 5 minutes to administer and may be given in either a self-report or interview format. The DAST may be used in a variety of settings to provide a quick index of drug abuse problems. The DAST-10 is a 10-item self-report instrument that has been condensed from the 28-item DAST. It was copyrighted in 1982 by Harvey Skinner, PhD and the Centre for Addiction and Mental Health, Toronto, Canada.

(OSDI)

Description
The OSDI, which was created by the Outcomes Research Group at Allergan Inc in order to quickly assess the symptoms of ocular irritation in dry eye disease and how they affect functioning related to vision. This 12-item questionnaire assesses dry eye symptoms and the effects it has on vision-related function in the past week of the patient’s life.[6] The questionnaire has 3 subscales: ocular symptoms, vision-related function, and environmental triggers. Patients rate their responses on a 0 to 4 scale with 0 corresponding to “none of the time” and 4 corresponding to “all of the time.” A final score is calculated which ranges from 0 to 100 with scores 0 to 12 representing normal, 13 to 22 representing mild dry eye disease, 23 to 32 representing moderate dry eye disease, and greater than 33 representing severe dry eye disease. The OSDI assesses quality of life measures, which aligns the questionnaire with Federal Food and Drug Administration’s emphasis on utilizing patient-reported outcomes in order to support medicinal product submissions and clinical drug trial efficacy. Tear break up time is another way that clinicians assess dry eye disease. When OSDI and tear break up time were analyzed, a statistically significant inverse correlation between the test scores was found; the higher or worse the OSDI score, the lower or worse the tear break up time was

Description
The SRI scale is a multidimensional instrument comprised of 49 items, and seven subscales including: Respiratory Complaints (SRI-RC), Physical Functioning (SRI-PF), Attendant Symptoms and Sleep (SRI-AS), Social Relationships (SRI-SR), Psychologic Well-being (SRI-WB), Anxiety (SRI-AX) and Social Functioning (SRI-SF). The SRI questionnaire assesses the patients’ quality of life during the preceding one week based on his/her level of agreement rated using a five Likert-scale from “strongly agree” to “strongly disagree”. Typically, subscale scores are generated or a total scale score is generated (SRI-SS) yielding a score ranging between 0 and 100, with higher scores indicating a better quality of life.

The Childbirth Care Satisfaction Survey (CCSS), had 10 items. Instead of the 5-point scale in the original PPS, 7 points were used to match the scales of the SSQ. Compared with the 5-point scale, the 7-point scale was believed to provide a more accurate and sensitive measure of a participant’s evaluation. The survey had other items related to participants’ educational level, employment status, number of babies in this delivery, number of previous deliveries, and type of delivery.

Description
The physical activity questionnaire for older children (PAQ-C), developed by Kowalski, Crocker & Donen (2004), is a seven-day recall self-administered questionnaire to evaluate moderately to vigorous physical activity in children belonging to the 8–14 years age group. The PAQ-C employs memory cues to facilitate the participant’s recollection of their physical activities, which makes the PAQ-C an apt tool to use with children.
