Description
The Neuropathic Pain Questionnaire-Short Form (NPQ-SF) is the shortest diagnostic tool for the assessment of neuropathic pain, designed with the goal to differentiate between neuropathic and nonneuropathic pain. The NPQ-SF consists of 3 items assessing tingling pain, numbness, and increased pain due to touch.

Description
The Short-Form McGill Pain Questionnaire (SF-MPQ) is used to assess sensory, affective, and evaluative dimensions of pain.It consists of a 15-item checklist assessing the sensory dimension (throbbing, shooting, stabbing, sharp, cramping, gnawing, hot-burning, aching, heavy, tender, and splitting) and affective dimension (tiring-exhausting, sickening, fearful, and punishing-cruel) of the pain experience.

Description
A quantitative measurement of the components of illness representations, as described by Leventhal’s Common-Sense Model (CSM) of selfregulation (Leventhal et al 1984, 1997). It is divided into three sections: identity subscale (14 symptoms), causal subscale (18 causes), and a third section which contains 7 subscales, including consequences, timeline acute/chronic and cyclical, personal and treatment control/cure, illness coherence, and emotional representations.

Description
A measure of manual ability for children with upper limb impairments. The scale measures a person's ability to manage daily activities that require the use of the upper limbs, whatever the strategies involved. It has been validated in cerebral palsy children (age 6-15). The Parent is asked to fill in the questionnaire by estimating their child's ease or difficulty in performing each activity when the activities are done without help, irrespective of the limb(s) the child actually use and whatever the strategies used to perform the activity. Note that the child is never asked to perform the activities in front of the evaluator.
