Description
A standard method for professionals to measure a child’s sensory processing abilities and to profile the effect of sensory processing on functional performance in the child’s daily life. This profile is designed for children from birth to 36 months and applicable for children with all types of disabilities and severity levels. It is a judgment-based caregiver questionnaire and each item describes children’s responses to various sensory experiences. Certain patterns of performance are indicative of difficulties with sensory processing and performance. When combined with other evaluations, observations, and reports, results of the Infant/Toddler Sensory Profile provide another perspective on the child’s strengths and challenges for diagnostic and intervention planning. Description
The DN4 includes a 4-section, 10-item checklist assessing neuropathic pain. Seven items are associated with the characteristics of pain (burning, painful cold, electric shocks) and pain associated symptoms (tingling, pins and needles, numbness, itching), and three items are related to neurological examination in the area of the pain (hypoesthesia to touch, hypoesthesia to prick, brushing).

Description
Is used widely for the evaluation of distal symmetrical peripheral neuropathy in diabetes. The MNSI includes two separate assessments, a 15-item self-administered questionnaire that is scored by summing abnormal responses, and a lower extremity examination that includes inspection and assessment of vibratory sensation and ankle reflexes and is scored by assigning points for abnormal findings.

FIVE

Description
FIVE has three versions (adult, child and parent). The adult version consists of four subscales (Fears about Contamination and Illness, Fears about Social Distancing, Behaviors Related to Illness and Virus Fears and Impact of Illness and Virus Fears) with 35 items in total. In the first two subscales, participants were asked to rate the frequency of feeling fear during the last week on a scale of four (1 for “I am not afraid of this at all” and 4 for “I am afraid of this all of the time”). The third subscale includes rating the frequency of doing specific behaviors within the last week on a scale of four (1 for “I have not done this in last week” and 4 for “I did this all the time last week”). The respondents were asked in the last subscale to rate the truthfulness of each statement on a scale of four (1 for “not true for me at all” and 4 for “definitely true”). The first two subscales (items 1–19) are directly related to fears, the third subscale (items 20–33) is related to behaviors, and the fourth one (items 34–35) is related to functionality.

Description
QOLIE-31 is the shortened form of QOLIE-89. QOLIE-31 is a specific instrument for rapid evaluation of the main QOL domains in adults with epilepsy. QOLIE-31 is a 31- item questionnaire organized in 7 subscales: seizure worry (SW), overall QOL (OQL), emotional well-being (EWB), energy and fatigue (EF), cognitive function (CF), medication effects (ME) and social functioning (SF).
