Description
The OQ®-45.2 is a self-report Outcome Measure designed for repeated measurement of client progress while in therapy and following termination. The OQ®-45.2 measures three subscales: 1. Symptom Distress (depression and anxiety) 2 Interpersonal Relationships (loneliness, conflict with others and marriage and family difficulties) 3 Social Role (difficulties in the workplace, school or home duties Features of OQ-45: Risk assessment: The OQ®-45.2 contains risk assessment items for suicide potential, substance abuse, and potential violence at work. Standardized data: The OQ®-45.2 is a standardized instrument with empirical support. It is based on normative data. Content that assesses functional level and change over time: The OQ®-45.2 measures functioning in 3 domains: Symptom distress (heavily loaded for depression and anxiety), Interpersonal functioning, and Social Role. Description

Description
A self-administered outcome measure of self-rated upper-extremity disability and symptoms.The items ask about the degree of difficulty in performing different physical activities because of the arm, shoulder, or hand problem (21 items), the severity of each of the symptoms of pain, activity-related pain, tingling, weakness and stiffness (5 items), as well as the problem's impact on social activities, work, sleep, and self-image (4 items). Each item has five response options. The scores for all items are then used to calculate a scale score ranging from 0 (no disability) to 100 (most severe disability). The score for the disability/symptom scale is called the DASH score.

Description
The NEI-VFQ is a broad, vision-targeted questionnaire used in the assessment of visual impairment. It has initially been developed by the National Eye Institute in order to match the need for a new tool to assess the extent of visual impairment in patients with various chronic eye conditions of moderate to high severity. Description
The PEDI is used to assess functional capabilities and performance, monitor progress in functional performance, and evaluate therapeutic or rehabilitative progress. in children with disabilities between the ages of 6 months to 7½ years. Can be used in older children if their functional development is delayed. The original publication date of the PEDI was 1992 (Coster & Haley,1992). The design of the PEDI supports both discriminative purposes such as determination of eligibility for disability related services (with norm-referenced standard scores) and evaluative purposes such as determining change following intervention (with criterion referenced scores).

An inventory for personality traits devised by Cloninger et al.. It is closely related to and an outgrowth of the Tridimensional Personality Questionnaire (TPQ), and it has also been related to the dimensions of personality in Zuckerman's alternative five and Eysenck's models[2] and those of the five factor model. TCI operates with seven dimensions of personality traits: four so-called temperaments Novelty Seeking (NS) Harm Avoidance (HA) Reward Dependence (RD) Persistence (PS) and three so-called characters Self-Directedness (SD) Cooperativeness (CO) Self-Transcendence (ST) Each of these traits has a varying number of subscales. The dimensions are determined from a 240-item questionnaire. WOMAC

Description
set of standardized questionnaires used by health professionals to evaluate the condition of patients with osteoarthritis of the knee and hip, including pain, stiffness, and physical functioning of the joints. The WOMAC has also been used to assess back pain, rheumatoid arthritis, juvenile rheumatoid arthritis, systemic lupus erythematosus, and fibromyalgia. It can be self-administered and was developed at Western Ontario and McMaster Universities in 1982.measures five items for pain (score range 0–20), two for stiffness (score range 0–8), and 17 for functional limitation (score range 0–68).[ Physical functioning questions cover everyday activities such as stair use, standing up from a sitting or lying position, standing, bending, walking, getting in and out of a car, shopping, putting on or taking off socks, lying in bed, getting in or out of a bath, sitting, and heavy and light household duties. The questions on the WOMAC are a subset of the questions of the Hip disability and osteoarthritis outcome score (HOOS). Thus, a HOOS survey may also be used to determine a WOMAC score.
