Description
The Activities-Specific Balance Confidence Scale (ABC) is a self report measure that assesses an individual’s confidence in performing various ambulatory activities without falling. Patients rate their confidence in performing tasks on a scale that ranges from 0% to 100%. The ABC can be self-administered or administered via personal or telephone interview.

PEDI

Description
The PEDI is used to assess functional capabilities and performance, monitor progress in functional performance, and evaluate therapeutic or rehabilitative progress. in children with disabilities between the ages of 6 months to 7½ years. Can be used in older children if their functional development is delayed. The original publication date of the PEDI was 1992 (Coster & Haley,1992). The design of the PEDI supports both discriminative purposes such as determination of eligibility for disability related services (with norm-referenced standard scores) and evaluative purposes such as determining change following intervention (with criterion referenced scores).

Description
The National Institutes of Health Stroke Scale is a systematic assessment tool that provides a quantitative measure of stroke-related neurologic deficit. The original scale was developed at the University of Cincinnati (Brott et al, 1989) and subsequently modified for the NINDS rtPA trial . The current version was first published by Lyden et al (1994) .The NIHSS was originally designed as a research tool to measure baseline data on patients in acute stroke clinical trials. Now, the scale is also widely used as a clinical assessment tool to evaluate acuity of stroke patients, determine appropriate treatment, and predict patient outcome.

Originally developed by James F. Fries and colleagues at Stanford University in 1978, Developed as a comprehensive measure of outcome in patients with a wide variety of rheumatic diseases but now also used for other conditions and aging , Should be considered a generic rather than a disease-specific instrument. Its focus is on self-reported patient-oriented outcome measures, rather than process measures. The Full Five-Dimension HAQ, assesses a hierarchy of patient outcomes in four domains: 1) disability, 2) discomfort and pain, 3) drug side effects (toxicity) and 4) dollar costs.

Description
A self-administered outcome measure of self-rated upper-extremity disability and symptoms.The items ask about the degree of difficulty in performing different physical activities because of the arm, shoulder, or hand problem (21 items), the severity of each of the symptoms of pain, activity-related pain, tingling, weakness and stiffness (5 items), as well as the problem's impact on social activities, work, sleep, and self-image (4 items). Each item has five response options. The scores for all items are then used to calculate a scale score ranging from 0 (no disability) to 100 (most severe disability). The score for the disability/symptom scale is called the DASH score.

The NEI-VFQ is a broad, vision-targeted questionnaire used in the assessment of visual impairment. It has initially been developed by the National Eye Institute in order to match the need for a new tool to assess the extent of visual impairment in patients with various chronic eye conditions of moderate to high severity. It is composed of 12 subscales: general health (1 item), general vision (1 item), ocular pain (2 items), near activities (3 items), distance activities (3 items), vision-specific social functioning (2 items), vision-specific mental health (4 items), vision-specific role difficulties (2 items), vision-specific dependency (3 items), driving (3 items), color vision (1 item) and peripheral vision (1 item). captures the physical, emotional and social aspects of visual disability, Thus, it assesses all aspects of quality of life (QoL) .
