Description
The Knee Outcome Survey is a patient self report survey that includes an Activities of Daily Living Scale (ADLS) and a Sports Activity Scale (SAS). The ADLS is a 14 item scale that queries patients about how their knee symptoms effect their ability to perform general daily activities (6 items) as well as how their knee condition effects their ability to perform specific functional tasks (8 items).

Description
The extent the child is able to perform activities is assessed from the perspective of the caregiver. In quality of life questionnaires the experience of impairments, dysfunction or social handicaps can be formulated either in the actual mode (e.g. is the patient incontinent) or from the experienced burden perspective (e.g. to what extent is the patient able to control the discharge of urine and faeces voluntarily). In the CDS the level of care dependency is formulated from the experienced burden perspective.

Description
A tool to assess parenting practices and skills, consisting of 42 items. Thirty five are divided into five subscales: Positive Parenting, Parental Involvement, Poor Monitoring/Supervision, Inconsistent Discipline, and Corporal Punishment. Description
The COOP/WONCA charts measure six core aspects of functional status: physical fitness, feelings, daily activities, social activities, change in health and overall health.
