Description
SEAMS is a 13-item self-reported medication adherence scale. Each item is scored on a 3-point Likert-scale where participants are asked to choose their level of confidence in taking medications correctly under different circumstances (1 = not confident, 2 = somewhat confident, and 3 = very confident). It was designed for patients with low literacy. The total score ranges from 13 to 39 where low scores indicate a low level of confidence and high scores indicate a high level of confidence.

Description
SOC-13 is a 13-item self-report measure developed by [Antonovsky., 1993] from the longer SOC-29 scale to briefly assess the extent of emotional control. The SOC-13, given its strong psychometric properties, was used extensively in several settings. All items are answered on a 7-point Likert scale over a semantic differential scale with two extreme anchoring phrases. Five items (namely, items 1, 2, 3, 7, and 10) are reversed in scoring. The total score would, hence, range between 13 and 91. Low scores indicate low/weak sense of coherence, and high scores mean high/strong sense of coherence. There are 5 comprehensibility, 4 manageability, and 4 meaningfulness items, however, the scale gives a single score of sense of coherence. Example items for comprehensibility include, “Do you have the feeling that you are in an unfamiliar situation and don’t know what to do?’’ and “Does it happen that you have feelings inside you would rather not feel?”. Items identifying manageability include, “Do you have the feeling that you are being treated unfairly?” and “Has it happened that people whom you counted on disappointed you?”. Meaningful items include, “Do you have the feeling that you don’t really care about what goes on around you?” and “How often do you have the feeling that there’s little meaning in the things you do in your daily life?”. To clarify further, the comprehensibility (consists of items: 2, 6, 8, 9, and 11), manageability (consists of items: 3, 5, 10, and 13), and meaningfulness (consists of items: 1, 4, 7, and 12).

Description
The SRI scale is a multidimensional instrument comprised of 49 items, and seven subscales including: Respiratory Complaints (SRI-RC), Physical Functioning (SRI-PF), Attendant Symptoms and Sleep (SRI-AS), Social Relationships (SRI-SR), Psychologic Well-being (SRI-WB), Anxiety (SRI-AX) and Social Functioning (SRI-SF). The SRI questionnaire assesses the patients’ quality of life during the preceding one week based on his/her level of agreement rated using a five Likert-scale from “strongly agree” to “strongly disagree”. Typically, subscale scores are generated or a total scale score is generated (SRI-SS) yielding a score ranging between 0 and 100, with higher scores indicating a better quality of life.

(SF-LDQ)

Description
The SF-LDQ consists of eight items that assess for the presence and severity of dyspepsia by measuring the frequency and severity of upper abdominal pain/discomfort, heartburn, regurgitation and nausea. It was developed from the Leeds Dyspepsia Questionnaire (LDQ), a longer form survey. Possible scores range from 0 to 32 with higher values corresponding with increasing severity of dyspepsia. The developers of the SF-LDQ have defined a score of 0 as 'no dyspepsia a score of 1 - 4 as 'very mild dyspepsia, a score of 5 - 8 as 'mild dyspepsia, a score of 9 - 15 as 'moderate dyspepsia' and a score >15 as indicative of 'severe dyspepsia'.
