Description
Tool for assessing health status in children with juvenile rheumatoid arthritis. Evaluates activities of daily living (ADLs) considered by expert opinion to be important and often difficult) for children with arthritis. The childhood HAQ has been adapted from the Stanford Health Assessment Questionnaire. At the present time, only the sections on disability and discomfort and pain have been adapted for use in children. Disability is measured in the categories of dressing, arising, eating, walking, hygiene, grip, and activities. Discomfort is determined by the presence of pain and its severity. It may be is either parent- or self-administered. For each of these categories parents are asked to record the amount of difficulty their child may have. Parents are requested to note only those difficulties that are caused by arthritis.

Description
A self-report measure of children's participation in recreation and leisure activities outside of mandated school activities. The CAPE provides information about five dimensions of participation. This includes diversity (number of activities done), intensity (frequency of participation measured as a function of the number of possible activities within a category), and enjoyment of activities. It also provides information about the context in which children and youth participate in these activities (i.e., with whom and where they participate).

Description
Instrument for assessing the presence and severity of depressive symptoms in children and adolescents (age 7-17).Has been used as a quick screening tool for depressive symptoms in clinical and general populations.There are 27 items quantifying symptoms such as depressed mood, hedonic capacity, vegetative functions, self-evaluation, and interpersonal behaviors. Each item consists of three statements graded in order of increasing severity from 0 to 2; children and adolescents select the one that characterized their symptoms best during the past 2 weeks. The item scores are combined into a total depression score, which ranges from 0 to 54. A higher CDI score means a higher depressive state.

Description
A rating scale for assessing severity of depression and change in depressive symptoms for clinical research trials in children and adolescents with depression. The CDRS-R, which was based on the adult Hamilton Depression Rating Scale, was originally developed as a rating scale for children aged 6–12 years. It is a 17-item scale, with items ranging from 1 to 5 or 1 to 7 (possible total score from 17 to 113), rated by a clinician via interviews with the child and parent. A score of ≥40 is indicative of depression, whereas a score ≤28 is often used to define remission (minimal or no symptoms).

Description
CART-Q was used to assess how athletes believe the coach perceives the athletic relationship with them. The questionnaire consists of 11-item meta-perspective items and 3 sub-dimensions: meta-closeness (4 items; e.g., “My coach likes me”), meta-commitment (3 items; e.g., “My coach is committed to me”), and meta-complementarity (4 items; e.g., “My coach is responsive to my efforts during training”). The direct version of the questionnaire also includes 11 direct-perspective items (3 items for direct-closeness; e.g., “I like my coach”; 3 items for direct commitment; e.g., “I am committed to my coach”; 3 items for direct complementarity; e.g., “When I am coached by my coach, I am responsive to his/her efforts”). For this study, the factorial structure of the Arabic version of the CART-Q was used in Kuwait and was prepared with both direct and meta perspectives. The items were assigned a score ranging from 1 (strongly agree) to 7 (strongly disagree) with a mid-point of 4 (half-way).

Description
The World Health Organization (WHO) and the International League Against Rheumatism (ILAR) initiated the Community Oriented Program for the Control of Rheumatic Diseases (COPCORD) project in 19814 . The aim of the program was to collect data on the burden of illness brought about by rheumatic disease and to institute preventive measures such as educational interventions. A number of countries have participated in this program and have used the WHO-ILAR COPCORD Core Questionnaire (CCQ) to determine the prevalence rates of rheumatic diseases in their countries. The English version of the CCQ includes questions on 7 domains: pain, symptoms (tenderness, swelling, and stiffness), trauma, functional ability, coping, health-seeking behavior, and treatments received.
