Description
Originally developed by James F. Fries and colleagues at Stanford University in 1978, Developed as a comprehensive measure of outcome in patients with a wide variety of rheumatic diseases but now also used for other conditions and aging , Should be considered a generic rather than a disease-specific instrument. Its focus is on self-reported patient-oriented outcome measures, rather than process measures. The Full Five-Dimension HAQ, assesses a hierarchy of patient outcomes in four domains: 1) disability, 2) discomfort and pain, 3) drug side effects (toxicity) and 4) dollar costs.

10/66 DRG diagnostic assessment for dementia

Description
An integrated one-phase dementia diagnostic battery comprising 1) a cognitive test battery; the Community Screening Instrument for Dementia (CSI-D)18 and the Consortium to Establish a Registry of Alzheimer’s Disease (CERAD) animal naming test and modified 10 world list learning, 2) a structured clinical interview; the Geriatric Mental State (GMS), which applies a computerized algorithm (AGECAT) to identify organic brain syndrome (dementia), schizophrenia, neurotic and psychotic depression, and anxiety neuroses, 3) the CSI-D informant interview. The 10/66 DRG assessment was designed to diagnose people with dementia living in the community, where the informant is typically a non-medical relative or a friend.

Description
A brief screening assessment for dementia based on an interview with an informant, usually a family caregiver. A screening tool for dementia in community and clinical settings. The informant rates the changes in cognitive functioning of the participant over the time period of 1 year or more. Each question of the IQCODE is rated on a 5-point scale from 1 (‘much improved’) to 5 (‘much worse’), with 3 representing ‘no change’. The sum of scores is averaged over the 16 items to give a score of 1–5.

Description
The Structured Telephone Interviews Questionnaire on Chronic Pain consists of a screening interview and a structured in-depth-interview questionnaire. The screening interview lasts approximately 5 minutes and consists of 12 questions to gather data on age and gender of participants and the duration of pain, frequency of pain during the previous week, intensity of pain during last pain episode, location of pain, and cause of pain. The structured in-depth-interview lasts approximately 23 minutes and consists of 44 questions to gather information about demographics, frequency, duration, intensity, and impact of pain on work and quality of life. In addition, the structured in-depth interview questionnaire gathers information about treatments, care pathways and the participants’ attitudes to their pain and its treatment, and their perceptions of the attitudes of their family, friends, colleagues, and doctors to their pain and its treatment. Description
The Brief IPQ was designed to rapidly assess cognitive and emotional representations of illness . It was developed by using one question that best summarises the items contained in each subscale of the Illness Perception Questionnaire-Revised which has over 80 items. The Brief IBQ comprises 5 items on cognitive representation of illness perception: consequences, timeline, personal control, treatment control, and identity. There are 2 items on emotional representation: concern and emotions. One item is on illness comprehensibility. The last item is on perceived cause of illness, in which respondents list the three most important causal factors in their illness
