Description
The OQ®-45.2 is a self-report Outcome Measure designed for repeated measurement of client progress while in therapy and following termination. The OQ®-45.2 measures three subscales: 1. Symptom Distress (depression and anxiety) 2 Interpersonal Relationships (loneliness, conflict with others and marriage and family difficulties) 3 Social Role (difficulties in the workplace, school or home duties Features of OQ-45: Risk assessment: The OQ®-45.2 contains risk assessment items for suicide potential, substance abuse, and potential violence at work. Standardized data: The OQ®-45.2 is a standardized instrument with empirical support. It is based on normative data. Content that assesses functional level and change over time: The OQ®-45.2 measures functioning in 3 domains: Symptom distress (heavily loaded for depression and anxiety), Interpersonal functioning, and Social Role. It enables the clinician to assess functional level and change over time.

Description
A self-administered eight-item questionnaire that was developed as a tool to measure subjective sleepiness in adults. The ESS asks the respondent to rate on a 4-point scale (0-3) their usual chances of having dozed off or fallen asleep while engaged in eight different activities that differ widely in their somnificity. The ESS specifically distinguishes reports of dozing behaviour (and estimates of SSPs) from feelings of fatigue and drowsiness/sleepiness, in the sense of ‘weariness from exertion’.

Description
The IPQ was developed to measure illness perceptions in a variety of illnesses. This questionnaire assesses perceptions on each of the five dimensions by asking patients for their own beliefs about their condition; The major cognitive components identified from research are: Identity - which is comprised of the label of the illness and the symptoms the patient views as being part of the disease; Cause - personal ideas about aetiology which may include simple single causes or more complex multiple causal models; Time-line - how long the patient believes the illness will last. These can be categorised into acute, chronic or episodic; Consequences - expected effects and outcome of the illness; and Cure/control - how one recovers from, or controls, the illness.

The concept of marginality describes the social margins that bring about social isolation as well as the psychological isolation of individuals. The Structured Telephone Interviews Questionnaire on Chronic Pain consists of a screening interview and a structured in-depth-interview questionnaire. The screening interview lasts approximately 5 minutes and consists of 12 questions to gather data on age and gender of participants and the duration of pain, frequency of pain during the previous week, intensity of pain during last pain episode, location of pain, and cause of pain. The structured in-depth-interview lasts approximately 23 minutes and consists of 44 questions to gather information about demographics, frequency, duration, intensity, and impact of pain on work and quality of life. In addition, the structured in-depth interview questionnaire gathers information about treatments, care pathways and the participants’ attitudes to their pain and its treatment, and their perceptions of the attitudes of their family, friends, colleagues, and doctors to their pain and its treatment. The AUDIT was developed as a simple method of screening for excessive drinking and to assist in brief assessment. It can help identify excessive drinking as the cause of the presenting illness. The Brief IPQ was designed to rapidly assess cognitive and emotional representations of illness . It was developed by using one question that best summarises the items contained in each subscale of the Illness Perception Questionnaire-Revised which has over 80 items. The Brief IBQ comprises 5 items on cognitive representation of illness perception: consequences, timeline, personal control, treatment control, and identity. There are 2 items on emotional representation: concern and emotions. One item is on illness comprehensibility.
