Description

A screening tool that identifies patients at risk for low health literacy. It is easy and quick to administer, requiring just three minutes. Based on a nutrition label from an ice cream container, patients are given the label and then asked 6 questions about it. In clinical settings the test allows providers to appropriately adapt their communication practices to the patient’s health literacy level. Researchers may use the instrument to measure health literacy and evaluate the impact of low health literacy on a variety of health outcomes.( Pfizer Inc.)

A self-administered eight-item questionnaire that was developed as a tool to measure subjective sleepiness in adults. The ESS asks the respondent to rate on a 4-point scale (0-3) their usual chances of having dozed off or fallen asleep while engaged in eight different activities that differ widely in their somnificity. The ESS specifically distinguishes reports of dozing behaviour (and estimates of SSPs) from feelings of fatigue and drowsiness/sleepiness, in the sense of ‘weariness from exertion’.

Description

The IPQ was developed to measure illness perceptions in a variety of illnesses. This questionnaire assesses perceptions on each of the five dimensions by asking patients for their own beliefs about their condition; The major cognitive components identified from research are: Identity - which is comprised of the label of the illness and the symptoms the patient views as being part of the disease; Cause - personal ideas about aetiology which may include simple single causes or more complex multiple causal models; Time-line - how long the patient believes the illness will last. The Structured Telephone Interviews Questionnaire on Chronic Pain consists of a screening interview and a structured in-depth-interview questionnaire. The screening interview lasts approximately 5 minutes and consists of 12 questions to gather data on age and gender of participants and the duration of pain, frequency of pain during the previous week, intensity of pain during last pain episode, location of pain, and cause of pain. The structured in-depth-interview lasts approximately 23 minutes and consists of 44 questions to gather information about demographics, frequency, duration, intensity, and impact of pain on work and quality of life. In addition, the structured in-depth interview questionnaire gathers information about treatments, care pathways and the participants’ attitudes to their pain and its treatment, and their perceptions of the attitudes of their family, friends, colleagues, and doctors to their pain and its treatment.
